Aotearoa Patient Advocates accepts three types of membership:
"Patient Membership": For patients who require clinical cannabis. Patients should identify one or more "Caregiver Members" if they require caregiver assistance for preparation of the clinical cannabis.
"Caregiver Membership": For caregivers who need to be able to provide the clinical cannabis to a "Patient Member" who is unable to provide it for themselves and/or who requires assistance in preparation of the clinical cannabis for consumption. "Caregiver Members" must identify one or more patient members for whom they are responsible.
"Supporter Membership": For good samaritans who support and promote the mission and aims of Aotearoa Patient Advocates.
Applying for Patient Membership:
Aotearoa Patient Advocates accepts "Patient Memberships" from patients with conditions where cannabis is recognised to be therapeutic. Anyone wishing to apply to become a "Patient Member" must provide verification of their condition. We also encourage you to obtain the signature of a supporting medical professional and will assist you in this if necessary.
If you, or a member of your family, wishes to apply to become an Aotearoa Patient Advocates "Patient Member," please print out and complete the Application for Patient Membership and if possible have your doctor complete the Medical Support Form. If you need assistance in having a doctor complete the Medical Support Form please contact us.
Once completed, return the forms either by post or in person to Aotearoa Patient Advocates so we can process your application and, once approved, issue you with a Patient Member’s certificate. This certificate verifies that you are a legitimate medical cannabis user who has the written support of your doctor or specialist.
If you have concerns about the prospect of discussing cannabis with your doctor, then you might like to talk to one of our members who has already been through the process and can provide support and reassurance.
When talking to your doctor it is very helpful to have the necessary resources available to support your application. Aotearoa Patient Advocates is more than happy to assist you in finding the best information to back up your case.
We also suggest printing out and giving your doctor a copy of our Mission Statement and Aims to look at.
